Admission Information

PRE-ADMISSION

Your physician, who is a member of our medical team, requested your admission to St. Luke's Health Brazosport.  Whenever possible, we will call you at home for all the preliminary information needed for admission.  Many of our questions are required by law for vital records, and others are of importance to the various departments of the health system that will be caring for you.  Please be assured that all information collected will be held in confidence.
 

PATIENT ACCESS

Our friendly and courteous Patient Access Team is here to serve your registration needs whether you are here for a simple lab test, x-ray test, same day surgery, or inpatient services.  Our Team is responsible for setting up your visit information so that our healthcare team has basic information to properly care for you.  The registrars may request identification and will obtain general written consent for treatment.  In addition, registrars collect financial information from you so that we may accurately bill you or your insurance for services rendered.
 

INSURANCE

IF YOU HAVE HEALTH INSURANCE

We will need a copy of your insurance card and forms that are supplied by your employer or the insurance company. You will be asked to assign benefits from the insurance company directly to the hospital.
 

IF YOU ARE A MEMBER OF AN HMO OR PPO

Your plan may have special requirements, such as a second surgical opinion or pre-certification for certain tests or procedures. It is your responsibility to make sure the requirements of your plan have been met. If your plan’s requirements are not followed, you may be financially responsible for all or part of the services rendered in the hospital. Some physician specialists may not participate in your healthcare plan and their services may not be covered.
 

IF YOU ARE COVERED BY MEDICARE

We will need a copy of your Medicare card to verify eligibility and process your Medicare claim. You should be aware that the Medicare program specifically excludes payment for certain items and services, such as cosmetic surgery, some oral surgery procedures, personal comfort items, hearing evaluations and others. Deductibles and co-payments are also your responsibility.
 

IF YOU ARE COVERED BY MEDICAID

We will need a copy of your Medicaid card. Medicaid also has payment limitations on a number of services and items. Medicaid does not pay for the cost of a private room unless medically necessary.
 

IF YOU DO NOT HAVE INSURANCE

A financial counselor from the Business Office will discuss financial arrangements with you. A hospital representative from the Business Office is also available to assist you in applying for Medicaid or other government assistance programs. We also have a financial assistance program available; please contact the Business Office for further information.

 

ACCOMMODATIONS

The hospital provides private and semi-private accommodations. We will try to honor your preference but there may be times when alternate arrangements may be necessary due to high occupancy, physician orders or other special reasons. We will honor your specific request for transfers when your preferred accommodations become available. The hospital’s primary responsibility is the medical care of patients; therefore, it may be necessary to move you to a different patient room after you are hospitalized.
 

OUTPATIENT PRE-REGISTRATION (Central Scheduling)

Should your physician choose to schedule outpatient services here at St. Luke's Health Brazosport, you can assist in your registration process by calling (979) 299-2888. This pre-registration process can be done for any testing or procedures that your physician orders here at St. Luke's Health Brazosport. Please have your insurance card and your physician’s orders ready when you call. We will collect and verify your demographic information as well as insurance information and will notify you of any estimated monies due on your date of service. When you arrive at the hospital, you will be fast-tracked through the Outpatient Registration Department located in the Main Lobby and directed to your procedure area.

PRICING TRANSPARENCY

 

We want to empower you with valuable information to help you and your family make important healthcare decisions. With this tool, you'll be able to find the general price estimate of the procedure you plan to have. Keep in mind that procedure costs can vary and do not include costs charged by others outside the hospital who are involved in a patient’s procedure, such as physicians, radiologists, and anesthesiologists.
