Pre-Admission Process

You may pre-register at the admissions department located within the hospital or you can complete the process over the telephone. Please note that you will need to supply the following during the pre-admission process:

  • Insurance information (If patient is covered under someone else's insurance, the clerk will need that person's name, date of birth, social security number and employer's information.)
  • Drivers License or Photo ID
  • Social Security Card or Social Security Number
  • Date of Birth
  • Phone Number
  • Mailing Address

Please schedule a pre-admission appointment with the hospital. We will need to obtain any needed blood tests, EKG’s and/or X-rays. Additionally, you will meet with a nurse regarding your health history, surgical permit and pre-op and post-op education. You also may be asked to speak to an Anesthesiologist.
During this time you will need the following:

  • Physician Orders for your Surgery or Procedure
  • A List of All Current Medications
  • A List of Allergies/Medical Conditions
  • A Copy of Any Advanced Directives or Power of Attorney

Day of Surgery

When ready, you will be taken to your designated unit for your surgery or procedure. Most IV therapies and outpatient procedures are performed in Day Surgery. Family members will be directed to the Surgery waiting room where they will wait until your surgery/procedure is completed. At that time, they will have the opportunity to speak to your surgeon or doctor for an update on your condition.

  • Enter through the main lobby of the hospital.
  • Report to the front desk at your given time.
  • When needed, you will be escorted back to the Day Surgery Unit.
  • Wear loose comfortable clothing and be sure to bring someone to drive you home after your surgery or procedure.
  • You will be prepped and prepared for your surgery/procedure.

Post Surgery

The final step of your journey will depend on what type of surgery/procedure you had performed. You will be taken to one of the following locations:

  • Recovery Room
  • Admitted to a hospital floor
  • Back to Day Surgery
  • Discharged and sent home for recovery

Stroke Care

With board certified neurologists and neurosurgeons on staff, CHI St. Luke's Health-Memorial is the area's first Joint Commission Certified Primary Care Stroke Center. The Stroke Center is dedicated to the diagnosis, treatment, prevention, rehabilitation, and recovery of stroke patients. The American Heart Association/American Stroke Association have recognized CHI St. Luke’s Health-Memorial with the organization’s highly coveted Get With The Guidelines®-Stroke Gold Plus Quality Achievement Award for outstanding stroke care. Additionally, in 2017, Memorial was named one of America's Best Stroke Center by WomenCertified and was ranked in the Top 10% of the nation for Stroke Care by CareChex, a division of Comparion®, an independent quality ratings group.


Cardiovascular Care

With designated cardiovascular surgical suites and catheterization labs, CHI St. Luke's Health-Memorial is the area leader in heart health. CHI St. Luke’s Health-Memorial holds the highest level of chest pain certification in Lufkin. The American College of Cardiology (ACC) and the Society of Cardiovascular Patient Care (SCPC), who completed a merger in 2017 to improve and streamline quality improvement services, in collaboration with the American Heart Association awarded the hospital with the Chest Pain Center Accreditation with Percutaneous Coronary Intervention, or PCI. The achievement recognizes CHI St. Luke’s Health-Memorial as an outstanding facility when dealing with patients who present with heart attack symptoms.


Dining & Room Service

From steaming hot vegetables and savory meats to tasty sandwiches and juicy hamburgers, The Café can satisfy the hungriest of appetites. Located on the first floor of the Lufkin hospital and recently remodeled to promote a fine dining experience, The Café offers a wide variety of foods seven days a week.

Hours of Operation
7 a.m.- 2 p.m.

Guests can order an after-hours meal by purchasing a $5 voucher at The Café register during normal business hours. Vouchers do not expire and several can be purchased at one time. Call ext. 7156 between 2 p.m. and 6 p.m. to have your meal delivered to the patient care area of your choice (excluding ICU and PCU).

Additionally, The Café offers guests snack items and fountain/bottled beverages. Whether you are dining alone or with a large group, The Café can accommodate you and make your overall dining experience pleasant.

Room Service Dining

At Your Request–Room Service Dining® (AYR) program is an innovative, fully developed concept of meal order and delivery for hospital patients based on the long standing practice of room service in hotels. AYR is a program that allows patients to order personalized meals from a full scale, restaurant-style menu of dining options that adhere to physician-established nutritional guidelines.

The patient simply has to pick up the phone by their bed, and they will be connected to someone who will take their order and arrange for the delivery of the meal. The patient will be able to place a food order any time during normal cafeteria operating hours.
