Social Services

Members of our Social Services staff are available for patients and their families when it comes to obtaining financial and medication assistance, nursing home placement, psychiatric placement, or other post discharge needs.

Our professional staff consists of registered nurses and social workers who have many years of acute care experience in case management. The department utilizes an interdisciplinary approach to ensure the continuity of patients’ care among settings, organizations, and providers by communicating each patient’s plan of care, current discharge needs, and referrals and/or resources identified. The department maintains a working relationship with all departments in the organization in order to effectively plan the care of our patients.

Our staff is here to assist you Monday through Friday from 8 a.m. to 5 p.m.

For FY24, our Hospital is focusing on improving the safety and quality of our care.  Focused performance improvements include: Acute Care and Ambulatory Care Patient Experience, Sepsis Mortality, 7 Day Readmissions, Hospital Acquired Infections such as CAUTI, CLABSI, C. Difficile, Patient death or serious injury associated with a fall while being cared for in a healthcare setting, and Stage 3, Stage 4, and unstageable pressure ulcers acquired after admission/presentation to a healthcare setting.
