The Transitional Care Clinic at Baylor St. Luke's Medical Center

The Transitional Care Clinic (TCC) team’s goal is to ensure that your transition from hospital to home is successful.  We offer an appointment for congestive heart failure patients at Baylor St. Luke’s Medical Center within seven days of discharge.

An essential component of staying healthy and monitoring your heart failure is to:  understand your medical condition, learn about your medications, and recognize the importance of a balanced diet.

During your TCC visit, you receive heart failure education tips on living a healthier lifestyle and a review of current medications and vital signs (blood pressure, weight, etc.). The team also ensures subsequent follow-up visits were scheduled with your physician.
