What to Bring to the Hospital

No special preparations are needed for preadmission testing, but please bring the following:  

    Current insurance card and information and any co-pay that may be required

    Physician orders, surgical consent form, medical history and physical form and/or testing results, such as EKG, clearance or lab results

    A list of your allergies, and/or current medications you are taking, including over-the-counter drugs

    A copy of your Advance Directive/Living Will

Know the name of the procedure/surgery for which you are scheduled and have a basic understanding of the procedure and an explanation of any risks involved. Also know your medical history. 

    List of medications you are currently taking, including name, dosage, and times of day you are taking them

    List of allergies

    Personal hygiene items (toothbrush, toothpaste, deodorant, etc.)

    Robe and non-skid slippers or your favorite walking shoes

    Loose-fitting pants or shorts

    Walker or crutches if you are having orthopedic surgery

    Telephone number of a contact person

    Books, magazines, crossword puzzles, tapes, hobby items to occupy your time for brief periods

    CPAP mask if you have sleep apnea

What Not to Bring for Your Hospital Stay 

    Electrical appliances such as hair dryers or razors

    Valuables, including credit cards, significant amounts of money, or jewelry

    Babies or small children should not be brought to the hospital

    Medications from home
