Digital Requests

Please select the type of request you have and complete the required form. We may follow up if we are in need of additional information to complete your request. Standard turnaround time for all requests is 5 business days. If this is a request requiring immediate attention, please select the option for expedited turnaround on the appropriate form. Please be prepared to explain the need for expedited turnaround, including whether it is impacting patient safety, a compliance or regulatory issue, etc.

 

 

Submit an event request for employee events or classes to be published to St. Luke’s Intranet (The Source). This request form is for events only; all class updates or requests should be submitted via the External Website Request form.

 

 

Please send all requests to update a physician’s profile on the CHISLH website to the Credentialing Department.

 

 

For the use of the St. Luke’s Health Marketing & Communications Department to submit requests for new marketing campaigns to be completed by the external advertising agency. The request will be reviewed by marketing leadership before approval of the project.

 

 

Submit any updates or changes needed on the St. Luke’s Health external website (StLukesHealth.org), including service line or location page updates. Updates to physician profiles (Find a Doctor pages) should be submitted to the Credentialing DepartmentWebsite requests related to location pages including naming and appearance could be delayed for the next 30 days while site optimizations are being conducted with CommonSpirit. 

If this is a class or seminar update, please provide the type of class, date, time, and location for each individual class.

 

 

Please send all form requests related to changing recipients, pulling submissions, form issues, new forms, etc. to the team managing all external web forms. 

 

 

Submit you intranet update for The Source here. Updates to The Source are now being managed by Jorge Cruz, Division Manager of Web Development. Please complete the form linked above to officially submit a request.

Changes submitted here can only be made to iSource pages. Any Source2 pages will need to be submitted through a ServiceNow IT/S request. Using your Inside CHI username and password, log-in to: https://myitsupport.service-now.com/texas.

 

 

Submit new social media posts or Facebook events. Social media requests can be either organic or paid promoted posts.
