St. Luke’s Health (St. Luke’s) is teaming with Population Health Advisors (PHA), a part of Baylor Scott & White Health, to advance its work on creating models and systems of care that will enhance care delivery in the Houston market. St. Luke’s has been transitioning to a value-based approach to care centered on improving quality and managing the health of patient populations while simultaneously reducing costs. By collaborating with PHA, St. Luke’s will benefit from PHA’s expertise and infrastructure as they operationalize population health management and other quality improvement initiatives.

Baylor Scott & White Health is the largest not-for-profit health system in the state of Texas. Its staff has implemented a population health infrastructure to improve health outcomes that has resulted in reducing medical plan costs for the Baylor Scott & White Health North Texas Division Employee Health Plan.
