Memorial Health System of East Texas Case Management Recognizes Long Term Care Facilities

Area long term care centers were recently recognized by Case Management at Memorial Health System of East Texas.  The centers were recognized for their efforts to partner with the hospital to enhance the overall care for its patients.  Among those recognized were Trinity Mission Health and Rehabilitation of Diboll in the Lufkin service area, Timberwood Nursing and Rehabilitation in the Livingston service area and Colonial Pines Healthcare Center in the San Augustine service area.  These facilities were chosen for their work as patient advocates, financial stewards and clinical excellence.
