Jane chairs the hospital’s Medication Safety Committee. She also serves as a Pharmacy Department representative on the Patient Safety Committee, the Regulatory Compliance Team, the CS Diversion Prevention Committee and the Diabetes Collaborative Practice Team. Jane received her BS in Pharmacy from the University of Kansas, completed a two-year pharmacy practice residency at the University of Texas M.D. Anderson Cancer Center, and received her MS in Hospital Pharmacy from the University of Houston. Current memberships include the Medication Safety Officers Society, the Texas Society of Health-Systems Pharmacists, and the Gulf Coast Society of Health-Systems Pharmacists, for which she served on the Board of Directors. As a member of the American Society of Health-Systems Pharmacists, she also served as the Chairman of the Section Advisory Group on Medication Safety. Her practice interests include Medication Safety, Patient Safety and Quality Improvement.
Lucretia C. Davis, PharmD

Lucretia received a Bachelor of Science in Pharmacy from Texas Southern University and a post-baccalaureate Doctor of Pharmacy from Xavier University of New Orleans. She completed a Pharmacy Practice Residency at St. Luke’s Episcopal Hospital and subsequently received extensive training in critical care and cardiovascular surgery recovery. Lucretia worked as a clinical pharmacist/coordinator for two-and-a-half years at Cypress Fairbanks Medical Center where she developed and implemented medication protocols primarily in the area of critical care. She is currently a member of the American Society of Health-System Pharmacists. Cardiovascular and toxicologic emergencies are among Lucretia’s interests.
Punit Hinsu, PharmD, MBA, MPH

Punit completed his training and began his career as a pharmacist in the Greater Philadelphia area. He earned his Doctor of Pharmacy degree in 2009 from Temple University and his Master of Business Administration with a concentration in Managerial Finance from La Salle University in 2011. He furthered his training in a Clinical Research and Drug Development Fellowship program with Fresenius Medical Care (Waltham, MA) and the Massachusetts College of Pharmacy and Health Sciences (Boston, MA). In 2013, he moved back to Philadelphia and received his Master of Public Health from Thomas Jefferson University in 2015. Later that year, he moved to Texas and joined the St. Luke’s Baylor Medical Center. He has since served as the Investigational Drugs pharmacist in charge of coordinating and overseeing clinical research protocol implementation and all Investigational Drugs activities. His practice interests include activities related to investigational drug services, including oncology research studies, gene therapy trials and specialty/personalized/precision medicine.

Katie received her Bachelor of Science in Biology at Texas Tech University in Lubbock, Texas and her Doctor of Pharmacy degree at The University of Texas at Austin College of Pharmacy in Austin, Texas. She completed her PGY1 acute care residency at NYU Langone Medical Center in Manhattan, New York. After her residency, Katie began as a clinical pharmacist I at St. Luke’s Baylor Medical Center for 3 years and then was promoted to a clinical pharmacist II specializing in cardiology/cardiovascular surgery. She is a member of ASHP.
Kady Phe, PharmD, BCPS
Clinical Pharmacy Specialist – Infectious Diseases

Kady received her Doctor of Pharmacy degree in 2012 from the University of Houston College of Pharmacy. She completed her PGY1 Pharmacy Practice Residency at St. Luke’s Baylor Medical Center and stayed on to complete a two-year Infectious Diseases Fellowship with St. Luke’s Baylor Medical Center and University of Houston College of Pharmacy. After completing her ID fellowship, Kady stayed on at St. Luke’s Baylor Medical Center and currently serves as a Clinical Pharmacist II in infectious diseases. She is a member of the Society of Infectious Diseases Pharmacist and the Infectious Diseases Society of America. Her practice interests include transplant ID and infections with multidrug-resistant organisms.
Zachary (Zack) Truman, PharmD
Clinical Pharmacy Specialist – Heart Transplant, LVAD & Mechanical Circulatory Support

Zack holds a Bachelor of Science in Biology from the University of Virginia in Charlottesville, VA and a Doctor of Pharmacy degree from the University of North Carolina at Chapel Hill Eshelman School of Pharmacy in Chapel Hill, NC. Zack completed a PGY1 Pharmacy Residency at the University of Chicago Medical Center before coming to St. Luke’s Baylor Medical Center for his PGY2 Cardiology Pharmacy Residency. Upon completion of his residencies, Zack stayed on at St. Luke’s Baylor Medical Center as a Clinical Pharmacist II in heart transplant, LVAD, and mechanical circulatory support. Zack is a member of the American College of Cardiology and ASHP.

Coty received her Bachelor of Science from Xavier University in New Orleans, LA and her Doctor of Pharmacy degree from Chicago State University in Chicago, IL. Thereafter, she completed a PGY1 pharmacy practice residency at Wesley Medical Center in Wichita, KS and completed a PGY2 in Solid Organ Transplant at Methodist-Le Bonheur University Hospital in Memphis, TN. Coty began her career at St. Luke’s Baylor Medical Center as a Clinical Pharmacist II in Kidney Transplant. She is a member of ASHP, ACCP, and American Society of Transplant.
Raymond Yau, PharmD

Raymond graduated from The University of Texas at Austin with a Doctor of Pharmacy degree in 2008 prior to completing a PGY1 Pharmacy Practice Residency at St. Luke’s Baylor Medical Center. After his residency, he gained experience in critical care as the MICU clinical pharmacy specialist at the University of Texas Medical Branch. He returned to St. Luke's Baylor Medical Center as a Clinical Pharmacist II in liver transplant in 2010. His interests include immunosuppression and transplant-related infections. He is a member of American Society of Transplant.
