Birth Certificates

The birth of a child is an exciting time and St. Luke’s Health–The Vintage Hospital is proud to be a part of such a significant milestone with you.

To help make the birthing process as seamless as possible, St. Luke's Health–The Vintage Hospital has provided the necessary birth certificate and registration forms available online for completion prior to coming to the hospital.

One of the many and most important responsibilities you have with the birth of your newborn is to complete the birth certificate, which is crucial to:

  • claim citizenship,
  • identify parental relationship (mother/father) and
  • attend school and work in the country.

New parents also need to complete the birth certificate packet and bring on the day of admission to the hospital.
