A birth certificate is an official government document that comes from the state. A birth record is an unofficial document that St. Luke's Health provides to new parents as a souvenir that can be suitable for framing. Copies of the birth record can be obtained through our Medical Record department, available by phone at (832) 355-2077.

To protect our patients' privacy, we don't transmit confidential patient information via e-mail. The best way to obtain copies of your records is to call our Medical Record department at (832) 355-2077 during business hours, Monday - Friday (8:00 a.m. - 4:30 p.m.). One of our representatives will be happy to assist you.
To obtain a copy of your medical records, you will need to complete this form. You can deliver the form in person to a St. Luke's Health Medical Records Department located at one of our hospitals, or you can mail it. IMPORTANT: There may be a charge for your records, so we strongly encourage you to call and speak to one of our representatives before mailing in the above form.
